Promo Membership Sale Application Form

I acknowledge that by writing and signing my name below I understand and accept the following:

That this is a limited time sale and that it will end when a target number of memberships have been sold.
That I am prepaying for an annual membership to secure a reduced rate and that it is gauranteed for two 1 year terms.
That the current timeline for the opening of this facility is in early 2025.
That construction timelines can change and that the opening could be delayed.
That I will be required to pay the annual membership cost in two equal payments.
That I will receive an invoice to pay the first installment on the 8th day after submitting this form.
(for example, if I submit it on Nov. 25, 2024, at 5pm, I will receive the invoice after 5pm on Dec. 2, 2024.)
That I am able to withdraw or cancel this application at any point within the first 7 days after I submit this application.
(to cancel my application I must email winnipegpickleballclub@gmail.com and indicate my name and request)
That failure to pay my invoice within 36hrs could result in the cancellation of my application and refusal of future membership
That I will not be charged the second installment until the club has been in operation for at least 5 months
That prior to using the courts I will obtain affiliation to the Pickleball Club of Winnipeg through Pickleball Manitoba ($15.00)
That prior to entering the Pickleball Club of Winnipeg I will read and sign the following documents:
- Waiver
- Member Code of Conduct
- Club Rules
